The first two that come to mind was the original Hibernian Dragon in DAoC, and the Sleeper in EQ. I actually like the idea of the nearly impossible boss with the secret strategy to defeat them, but on the other hand making them is probably considered a crime against humanity.

They may as well just copy and paste portions of the local phonebook into their patch notes, for all the good they do.
Sorry for the wall of text, but I mean they couldn't even get the dialogue camera to work right before launch, that sounds like an alpha/beta problem to me.
