I play DDO because of real time combat. I'm not sure about dolling up the combat for the camera to tilt on critical hit, it'd almost be a bad thing in PVP situations.

That sounds fine and all but imagine being locked in combat while someone from an opposing faction comes at you from behind.

The way it works in FFXI is that if and when you need to change targets you can, you simply tab (or use the mouse) to select your new target and hit enter.

Plus, a game like this would definitely be more of a PvE-style MMO. I could see doing PvP, but it wouldn't be the majority of the content.

I love this idea. I've been following The Agency and one of the concepts that they've talked about is "no more stone faces". They want to make the characters react to events, rather than just quickly change when you do an emote, then switch back to a stone face. I don't know how that will play out during combat, but I cannot wait to find out.
