This would be the opposite of what you're asking, but I think a Shadowrun MMO could be very successful. It's not Sci-Fi or Fantasy, but an awesome mix of the two. MMO's are about chances for heroism and adventure. You're just not going to find that unless the environment is fantastic, in a manner of speaking.

I'd say something along the lines of 1800s crime-fighting, being detectives, vigilantes, or what have you. Another option would be an exploration-based game, similar to Indiana Jones adventures, deep in the uncharted Amazon, the ruins hidden in the Sahara, the African bush... the list goes on.

You guys reported about a vampire vs slayers game just a few days ago.

There are several Pirate games. Pirates of the burning sea, Pirates of the Caribbeans, yohoho puzzle pirates.

And there are MMO (T)urn (B)ased (S)trategy, Bang! howdy.
MMO (R)eal (T)ime (St)rategy, Ballerium.

There are enough other choices you just have to look past the main stream. Even webbases wargames are MMO. Like playteq.com or Earth.

Something set in the Wild West, maybe a GTA Online like several people have suggested (though the logistics of that would be insane), some sort of espionage game... there are a lot of options. The problem is making them fun. :)

(To be fair, though, if you think about it - EVE Online really IS the Wild West, just in space. It already works. So just give us the great plains region to roam about in, some horses and wagons and sixguns, and let's go.)

In case you didn't know already, there is a new MMO being created that is based on Zombies and Horror. It is called Exanimus and its located at http://www.exanimusthegame.com. No more sci-fi or fantasy for me. I'm all about the zombies!
